Prevention and treatment of protein energy wasting in chronic kidney disease patients: a consensus statement by the International Society of Renal Nutrition and Metabolism.

چکیده
Protein energy wasting (PEW) is common in patients with chronic kidney disease (CKD) and is associated with adverse clinical outcomes, especially in individuals receiving maintenance dialysis therapy. A multitude of factors can affect the nutritional and metabolic status of CKD patients requiring a combination of therapeutic maneuvers to prevent or reverse protein and energy depletion. These include optimizing dietary nutrient intake, appropriate treatment of metabolic disturbances such as metabolic acidosis, systemic inflammation, and hormonal deficiencies, and prescribing optimized dialytic regimens. In patients where oral dietary intake from regular meals cannot maintain adequate nutritional status, nutritional supplementation, administered orally, enterally, or parenterally, is shown to be effective in replenishing protein and energy stores. In clinical practice, the advantages of oral nutritional supplements include proven efficacy, safety, and compliance. Anabolic strategies such as anabolic steroids, growth hormone, and exercise, in combination with nutritional supplementation or alone, have been shown to improve protein stores and represent potential additional approaches for the treatment of PEW. Appetite stimulants, anti-inflammatory interventions, and newer anabolic agents are emerging as novel therapies. While numerous epidemiological data suggest that an improvement in biomarkers of nutritional status is associated with improved survival, there are no large randomized clinical trials that have tested the effectiveness of nutritional interventions on mortality and morbidity.

Recent advances in the pathophysiology and management of protein-energy wasting in chronic kidney disease
Protein-energy wasting (PEW) is a syndrome that consists of metabolic and nutritional abnormalities that often occur in chronic kidney disease (CKD), and PEW has been found to be associated with increased morbidity and mortality. A review was conducted to identify publications detailing the pathophysiology and management of PEW in CKD. The International Society of Renal Nutrition and Metabolism...

Background: Chronic kidney disease (CKD) is a progressive condition that affects many aspects of patient’s life with adverse outcomes of kidney failure, cardiovascular disease (CVD), and premature death. Malnutrition is a relatively common problem in these patients that may be the result of inadequate intake, increased catabolism, or loss of nutrients in the dialysis. The aim of this study was ...
متن کاملRisk Stratification of Hemodialysis Patients with Protein-Energy Wasting Using Hand Grip Strength and Malnutrition-Inflammation Score: are Two Indices Better than One?
Background: We aimed to detect whether risk stratification of hemodialysis (HD) patients with a combination of both malnutrition-inflammation score (MIS) and hand grip strength (HGS) indices identified more precisely patients at increased risk of protein-energy wasting (PEW). Methods: This was a deductive-analytical cross-sectional study. We determined the HGS...

Among patients with ESRD (End-Stage Renal Disease) who receive peritoneal dialysis treatment, the prevalence of protein-energy malnutrition (PEM) is considerably high and it is regarded as a strong predictor of increased morbidity and mortality rates. Identified by the International Society for Renal Nutrition and Metabolism in 2008, the diagnostic criteria for of protein-energy wasting are cla...
متن کاملA proposed nomenclature and diagnostic criteria for protein-energy wasting in acute and chronic kidney disease.
The recent research findings concerning syndromes of muscle wasting, malnutrition, and inflammation in individuals with chronic kidney disease (CKD) or acute kidney injury (AKI) have led to a need for new terminology. To address this need, the International Society of Renal Nutrition and Metabolism (ISRNM) convened an expert panel to review and develop standard terminologies and definitions rel...
